Final Forms
McCord uses Final Forms to handle all paperwork for athletics. All parents and student athletes will be required to create an account and update information as needed. Students/parents must complete all forms on Final Forms before they can tryout out or practice for a team.
As a reminder, students will use their SYLVANIA SCHOOLS email to create their accounts. Also, make sure to choose the sport that you are playing that season. If you are a homeschooled student or attend a local private school, please use a personal email that will be used to access the forms. The link below provides specific directions to set up an account and complete the required documents.
