Below are some resources for parents and guardians whose preschool student is currently receiving special education services.

Preschool Special Education Assessment Process

If you suspect your preschool aged child has a disability or delay that impacts their learning or they have received a medical diagnosis and you would like to find out if the preschool program can provide services to them, you can request a preschool evaluation. They must be three to five years and not have begun kindergarten yet in order for the preschool team to evaluate them. Please note that requesting an evaluation does not mean that a child will automatically receive services. The evaluation process will determine if an educational disability is present or not.

If you would like to begin the evaluation process, please follow these steps:
  1. Call the preschool secretary, Veronika, at 419-824-8587 or send an email to [email protected] stating that you are requesting an evaluation. The email must include the following information: 
    • Child's Name

    • Child's Date of Birth

    • Parent/Guardian Name(s)

    • Full Address

    • Parent/Guardian Phone Number(s)

    • Parent/Guardian Email(s)

    • Brief description of area(s) of concern

  2. The secretary will then email you the assessment application and connect you with the preschool psychologists.

  3. The psychologist will email you paperwork to complete and schedule a home visit.

  4. If an educational disability is suspected, the child will be asked to complete a play assessment.

  5. The parent(s)/guardian(s) and the evaluation team will then meet to discuss the evaluation results.

  6. If the child is found eligible, an Individualized Education Plan (IEP) will be developed in order to provide the child with appropriate services.
